$27.280 - $34.100 - $40.920The stated pay scale reflects the range that Sharp reasonably expects to pay for this position. The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant's years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.
What You Will Do
The Substance Use Navigator (SUN) provides assessment and referral to outpatient clinics, for those patients experiencing substance use disorders, most particularly Opioid Use Disorder. The SUN conducts initial brief assessments, introduces patients to Medication assisted treatment (MAT) programs and services. Facilitates appointments at outpatient MAT clinics when appropriate.
Required Qualifications
- H.S. Diploma or Equivalent
Preferred Qualifications
- Bachelor's Degree in Social Work.
- 2 Years working in hospital setting and/or chemical dependency center.
- Certified Alcohol and Drug Counselor (CADC) - The California Certification Board of Alcohol and Drug Counselors -PREFERRED

Effectively and collaboratively receives referrals from medical providers and multidisciplinary team for patients with Substance Use Disorder.

Contributes to, and effectively communicates elements of discharge planning related to SUD treatment (MAT capable when applicable). - Discharge Planning
Schedules appointments at SUD (MAT capable when applicable) for ongoing treatment and address access barriers.
Provides referrals to other services such as shelters, primary health, mental health, social services, sober living and residential treatment.
Establishes a system that ensures patient referral and follow-up outside of the SUN's regular hours.
Obtains and maintains knowledge of insurance benefits and exclusions related to treatment.
Maintains knowledge of community clinics, services and limitations.
Maintains and continually assesses for changes to referral pathways to community clinics. - Documentation
Completes screening and intervention within hospital guidelines for timeliness.
Documents interventions with patients and post-discharge appointments in EMR.
Compiles monthly accounting of targeted metrics such as number of patients served, buprenorphine administered, prescriptions and referrals to care or any other facility determined metric.
Follows hospital and regulatory requirements for documenting and reporting abuse situations.
Knowledge, Skills, and Abilities
- Understanding of Substance Use Disorder (SUD) as a medical condition.
- MAT in treatment and harm reduction.
- Medical terminology knowledge.
- Knowledge of community MAT providers and clinics.
- Effective interpersonal and communication skills.
