Job Summary
Care Connect is a Multidisciplinary Care Management Team that collaborates for patient care within University of Michigan Medical Group (UMMG). These UMMG Ambulatory Care Units of Michigan Medicine supports the care and treatment of patients for a wide variety of Primary Care and Specialty needs. The multitude of clinics and programs within Michigan Medicine share a common philosophy of caring for the whole person which not only includes excellent medical care but an accompanying regard for the mental health and well-being of our patients and the families who love them. Psychosocial care is an indispensable component in the overall clinical care of these complex patients.
This 1.0 FTE Care Connect Float Social Work (MSW) position will be responsible for providing social work services to patients across Ambulatory Care areas and patients of all ages. This position will support the Care Connect Social Work teams as a Social Work (MSW) Float to cover PTO, leaves, and absences as well as assist the team to manage high acuity/high referral caseloads. The families and patients with health concerns and chronic diagnoses have intense ongoing psychosocial needs and multiple barriers to care. Additionally, data confirm the prevalence of mental health needs among patients and their caregivers.

Required Qualifications*
- A Master's degree in Social Work from a school accredited by the Council of Social Work Education.
- A current license to practice social work in the State of Michigan.
Desired Qualifications*
- Previous experience with medical and mental health social work.
- Skilled in utilizing mental health screening tools such as ASQ, C-SSRS, PHQ-9, GAD-7
- Skilled in utilizing elements of therapeutic modalities including CBT, DBT, ACT, MI, Mindfulness.
- Previous experience with issues of chronic illness, adjustment to illness and disability, learning/school and employment issues/, locating community resources
- Proficiency in adjustment, loss, and bereavement issues
- Skilled in developing rapport and effectively communicating with a diverse patient population
- Efficient, accurate assessment of patient needs and effective care planning
- Effective organizational skills, flexibility, self-motivation, the ability to effectively prioritize multiple competing tasks in a dynamic medical setting
- Independent, sound decision-making is necessary for success in this position
- Effective collaboration with multi-disciplinary teams
- Proficiency in keeping accurate and timely clinical records by computer
Work Schedule
This is a 40 hour/week position. The typical schedule will fall between Monday-Friday with start and end times 8:00-4:30 or 8:30-5:00.
Work Locations
This position is expected to be a hybrid position located remotely and the Ambulatory Care clinic where the Float Social Work support is needed. Work location is adjusted as needed to accommodate patient care needs/volume. Candidates need to be flexible and willing to adjust between hybrid and onsite work as needed.

Underfill Statement
This position may be underfilled at a lower classification depending on the qualifications of the selected candidate. Advancement through the career ladder at the Expert or Clinical Scholar Level is required for placement in the MSW Senior Classification.
