Overview
The Stroke and Epilepsy Program Coordinator is an exciting dual role providing clinical expertise and leadership within the interdisciplinary teams responsible for the Good Samaritan Stroke Program and Legacy Health Epilepsy Program. In the role of Good Samaritan Stroke Coordinator, collaborates with physician leadership and peers both locally and throughout the Legacy system to ensure all program certification standards are being met to optimize the outcomes for stroke patients. In the role of Epilepsy Program Coordinator, engages with patients and epilepsy program team members to ensure smooth coordination of care across the epilepsy services pathway. In both roles, support ongoing nursing education, program development, and initiatives to improve quality of care. Ideal candidates demonstrate strong interest in working with the neurological patient population, have experience in data collection and review, facilitating quality improvement, nursing education, and protocol/standard development and maintenance.
Responsibilities
GENERAL ACCOUNTABILITIES AND ESSENTIAL FUNCTIONS:
- Clinical expert resource for staff providing stroke patient care
- Effectively assess inpatients using neuroscience nursing; where appropriate, provide consultation, coordination and advocacy related to patient care in area of specialty.
- Empower clinical decision-making by promoting the development and use of critical thinking skills, technical skills, and interpersonal communication skills unique to this patient population.
- Provide timely feedback and coaching on stroke care protocols and site-specific processes to stroke care team members.
- Resource for questions about NIHSS, care protocols and complex stroke-related patient issues.
- Serve as clinical liaison to research team when research studies are conducted.
- Facilitate team coordination in the provision of evidence-based stroke care
- Coordinate and participate in multidisciplinary stroke rounds.
- Consult on and support the communication of the daily plan of care with all team members.
- Assess patient care processes and the outcomes of care; plan and intervene as appropriate to promote delivery of optimal stroke care.
- Participate in systemwide forums to standardize care protocols and processes and support system integration of best practices.
- Support staff and community education
- Support a standardized process and content for stroke education; review materials to ensure relevance and accuracy.
- Collaborate with bedside nurses to provide patient and family stroke education.
- Work together with LH Stroke Program, Nurse Managers, Clinical Nurse Specialists, and unit educators to assess needs and provide ongoing education to support staff competency in stroke patient care.
- Partner with other stroke team members to deliver community education around the prevention, recognition and treatment of stroke.
- Manage program data, evaluation and improvement efforts
- Work closely with the LH Stroke Program to ensure data is accurately abstracted and reported to support continuous improvement in stroke patient care and for compliance with LH Stroke Program protocols and regulatory requirements (e.g.,Core Measures, Get With the Guidelines)
- Be knowledgeable about hospital Stroke Program performance, goals, and areas of opportunity and support the communication of performance widely throughout the stroke care team and organization.
- Actively work with unit leaders, educators and other care team members in pursuit of performance excellence and improvement.
- Collaborate with the LH Stroke Program team to prepare the organization for disease-specific care certification surveys.
Qualifications
Education:
Academic degree in nursing (BSN or MSN/MN) required. Internal candidates promoting may meet the degree requirement with verification of bachelor's or master's degree in progress with an expected completion date within 2-years of the promotion date; progress will be reported periodically to the site Chief Nursing Officer (CNO).
Experience:
2-3 years of expertise in the provision and coordination of care for stroke patients required. Experience leading performance improvement efforts and disease-specific program management strongly preferred.
Skills:
Demonstrated skills in patient assessment, multidisciplinary collaboration, problem solving, data analysis, leading change, teaching, coaching, written and verbal communication. Proficiency in Microsoft Office Suite and clinical information systems.
