Description:
Coordinates and manages a patient's healthcare plan. They work with patients, families, and other medical professionals to ensure patients receive the best care possible.
1. Assessing new patients by gathering information. Reviewing diagnoses. Performs a part of the Welcome to Medicare meeting for new patients
2. Works with provider on the care plan and assisting to obtain the assistance the patient requires. Revises care plans based on patients' progress and input from providers.
3. Works with patients, providers and other medical professionals to schedule appointments, implement treatment plans and ensures the patients receive the care they need
4. Educates patients and their families about treatment options.
5. Provides other healthcare services such as providing symptom management and offering emotional support.
OTHER DUTIES
1. Enter, edit and update patient demographic and/or clinical information accurately to ensure quality data
2. Patient files:
* Maintain strict patient confidentiality
* Update immunization records and health maintenance flow sheets
* Enter all procedures done: Blood draws, vital signs, wound irrigations, injections, etc. and sign document with full last name and title
* Note all phone conversations in writing and sign
- Graduation from an accredited program of Nursing , either LVN or RN
- At least three (3) years Clinic Experience in out-patient care
- Possession of a valid certificate of registration as a Licensed Nurse
- Valid certificate of training in Cardio-Pulmonary Resuscitation
Compensation details: 34-48 Hourly Wage
