Job Summary

The Division of Hospital Medicine program is seeking to hire physician assistants (PA) or nurse practitioners (NP) to join our hospitalist program and practice inpatient medicine across multiple service lines, primarily for Medical Short Stay Unit. Specifically, we are seeking 1 provider to cover primarily evenings with some nights possible.

The Division of Hospital Medicine is a rapidly growing, dynamic and evolving group within the Department of Internal Medicine. The Division cares for complex internal medicine patients that have needs ranging from observation/short stay and planned procedures, to general care, moderate and intensive care including those positive for (or patients under investigation for) COVID-19. Clinicians in the Division care for patients with a wide range of illnesses and system-based problems, ranging from hematological malignancies to cardiovascular, gastrointestinal, pulmonary, and multi-system disease. Advanced practice providers (APPs) work in close collaboration with hospitalist attending physicians and resident trainees to assist with triaging patients, completing history and physicals and other daily documentation, evaluating, and managing patients who are admitted or need admission, and formulating diagnostic and treatment plans. The role also includes admitting, triage and cross-coverage responsibilities along with procedural and other specialty duties as described below.

The Hospital Medicine APP role is based on collaborative model of care teams made up of physician and advanced practice providers on services including (but not limited to): Medical Short Stay Unit, Medicine Faculty Hospitalists, Medicine Procedure Service, Medicine Hematology co-managed service, and internal medicine resident ward services. APPs in the Division of Hospital Medicine are expected to not only engage in clinical activities and responsibilities, but also engage in education and continuous quality improvement activities.

Responsibilities*

  1. Assess and triage patients referred for admission from external sources (e.g., clinics), the emergency department or other internal sources (e.g., other clinical services) for appropriateness and destination of care delivery.
  2. Assess new patients for relevant & complete health and medical history, perform an appropriate physical examination and complete documentation and procedures related to admission.
  3. Manage the care of admitted patients (MSSU, MFH, or ED Boarding Team), including the following: 
    1. Order appropriate diagnostic studies, medications, and treatments for acute conditions and manage co-existing conditions.
    2. Review, interpret, and manage diagnostic data. 
    3. Write prescriptions for scheduled and unscheduled medications under delegated prescriptive authority from the attending physician(s) as appropriate. 
    4. Competently perform invasive and non-invasive procedures, including but not limited to lumbar puncture, paracentesis, thoracentesis and joint aspirations as appropriate.
    5. Safely coordinate and oversee patient transfers between sites of care within the University and across external care sites.
    6. Coordinate and lead patient and family communication efforts, providing patient and family education, discussing goals of care, and delineating code status as needed.
    7. Call consultants and carry out recommended work up and plans of care as recommended by respective specialties.
    8. Lead and participate in safety huddles, multidisciplinary rounds, and shift sign-outs that are patient-centered.
  4. Perform general medicine consults, as well as rounding on established patients on the consult service in UH/UHS, with expansion to Pavilion in 2025.
  5. Manage patients admitted with COVID-19 in moderate care settings including the use of heated high flow oxygen, prone positioning, non-invasive ventilation (e.g., CPAP, BiPAP, and associated medications e.g., Precedex, low-dose vasopressors).
  6. Discharge patients, complete discharge documentation, and communicate discharge plans with other care providers as part of patient?s healthcare continuum.
  7. Lead and participate in case conferences and collaborative educational series.
  8. Orient and precept newly onboarding APPs, nurse practitioner students, physician assistant students, and medical students as appropriate.
  9. Maintain knowledge of medical principles, practices, standards, and techniques in accordance with Michigan Medicine policies and procedures, Medical Staff Bylaws and State and Federal laws/regulations. 
  10. Build and maintain professional relationships with physician other health care providers. 
  11. Seek consultation with faculty, as appropriate, for concerns and issues outside the one?s scope of knowledge or skills

    Nursing Specific Info

    Required qualifications must be met by the candidate in order to be interviewed and considered for the position. Posting may be filled after the initial 5-day posting period.

    Required Qualifications*

    Nurse Practitioners

    • Current licensure to practice as a Registered Nurse and Nurse Practitioner specialty certification in the State of Michigan.
    • Certification as an Adult Gerontology Acute Care Nurse Practitioner 
    • Graduate of an accredited Nurse Practitioner program or who has successfully completed all required Nurse Practitioner program coursework at an accredited Nurse Practitioner program to qualify to take their specialty certification exam.
    • Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN). 
    • Current DEA registration required
    • ACLS/BLS certification required (separate AHA approved BLS certification also required unless ACLS was completed through the UMHS Simulation Center; ACLS course taken through the UMHS Simulation Center will meet both the ACLS and the BLS requirement).
    • At least 1 year of clinical advanced practice provider employment experience.

    Physician Assistants

    • Satisfactory completion of a Physician Assistant program accredited by Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor agencies, CAHEA or CAAHEP.
    • Certification by the National Commission on Certification for Physician Assistants (NCCPA).
    • Current license to practice as a Physician Assistant in the State of Michigan.
    • State of Michigan Controlled Substance License Required  
    • Current DEA registration required
    • ACLS/BLS certification required (separate AHA approved BLS certification also required unless ACLS was completed through the UMHS Simulation Center; ACLS course taken through the UMHS Simulation Center will meet both the ACLS and the BLS requirement).
    • At least 1 year of clinical advanced practice provider employment experience.

    Desired Qualifications*

    • At least 2 years of advanced practice employment experience in inpatient internal medicine. 
    • Previous observation unit advanced practice employment experience. 
    • Intensive care unit or ED advanced practice employment experience.
    • Current employment as an advanced practice provider. 
    • Demonstrated ability to efficiently and effectively manage multiple clinical tasks with attention to detail. 
    • Demonstrated ability to communicate effectively (oral and written).
    • Demonstrated ability to excel in a team-based model of care. 
    • Demonstrated ability to role model professional behavior in all situations. 
    • Demonstrated high level of critical thinking. 
    • Demonstrated high level of motivation to learn and participate in diverse patient care activities. 
    • Able to work a flexible schedule to accommodate job requirements. 
    • Able to physically don and doff PPE required for droplet and airborne isolation. 
    • Ability to perform various diagnostic, therapeutic, and specialty procedures. 

    Work Schedule

    Shifts range from 10-12 hours in duration, and work hours include those that cover primarily evening with occasional night shifts. This includes possible cross coverage shifts to other hospital medicine services. Holiday and weekend coverage is required. On-call responsibility for 48-72 hours a month.

    Union Affiliation

    This position is covered under the collective bargaining agreement between the U-M and the Michigan Nurses Association and the U-M Professional Nurse Council union, which contains and settles all matters with respect to wages, benefits, hours and other terms and conditions of employment.

    Background Screening
